TOURISM

Visa application form
-
One recent passport style photo
-
Passport, valid for at least 3 months after the visa expiry date (you also need
to add a copy of first three pages of the passport and previous Shenghen visas)

Copy of the ticket, two way booking, or Green Card along with the car passport,
if traveling by car (original and copies).

Proof of accommodation in Italy:
- tourist agency voucher;
- hotel booking receipt;

invitation letter
from a person in Italy who has legal permission to stay there (permesso di
soggiorno) or a copy of identification documents if the person who is inviting
is a citizen of Italy.

Proof of financial means:
- travel checks;
- current bank slips along with the copy of the bank cards;
- bank or insurance policy if invited person can not provide a proof of
his(hers) financial means (originals and copies)

Any documents that can prove tourism purpose of the visit as well as
social-economical state of the applicant.

For those who work:
- a reference from the employer, translated in Italian or English, with
position, starting job date and salary mentioned. If a person is applying for a
long term visa, the reference should also state that the period of employee's
leave corresponds to the paid leave term, which he has the right to have.
- copy of the labor book;
- if the employer is a private entrepreneur, it is necessary to add a copy of
governmental registration, translated into Italian or English.

For private companies, owners of companies which are involved with
professional, economical, commercial activities:
- company's registration certificate and the license, translated into Italian
or English, with the type of activity mentioned, along with other documents
which prove the effectiveness of the company (accounting balance, invoices,
taxes paid, etc. (originals and copies)
- certificate of taxpayer
- inquiry from tax administration about received income and absence of debt to
budget

For students
- a proof note from the university that a student is really enrolled in the
study process at the moment and a transcript book, translated into Italian or
English;
- if financially dependant from a third party (for example parents), provide
documents as for the financial state of the third party.

For retired:
- a reference about receiving pension, translated in Italian or English, bank
statement (if applicable), other documents that can serve as a proof of
financial state and property state of the applicant (original and copy).

For schoolchildren
- reference from school
Economical state in Ukraine can be proven by the following documents:

property possession or other income note, translated in Italian or English
(originals and copies)
-
bank or credit cards statements;
-
fiscal document's declaration, translated into Italian or English
-
medical insurance, with a minimum of 30 000 euros coverage.
REQUIREMENTS FOR MINOR CHILDREN

If owns a passport - a permission to go to Italy, written by parents with the
name of accompanying party mentioned, translated into Italian or English.

In on parent's passport - a permission of another parent for a child to go to
Italy, translated into Italian or English.
-
If going with a third party - a permission to go to Italy, written by parents
with the name of accompanying party mentioned, translated into Italian or
English.
Please note, that notarized permission is valid for 6 months from date of issue.
TOURIST VISA FOR A SHORT TERM FAMILY TRIP

Besides the documents mentioned above, it is necessary to provide a certificate
that proves shows the relationship with people you are going to visit:

Birth certificate, marriage certificate, name change certificate;

If parents' last names differ, a certificate showing family ties must be
mentioned
Please note! All documents in Russian or Ukrainian must be translated
into Italian/ English.
PLEASE ATTACH TO A PHOTOCOPY OF ALL DOCUMENTS EXCEPT THE VISA APPLICATION FORM
ALONG WITH YOUR VISA APPLICATION FORM.
THE EMBASSY RESERVES THE RIGHT TO REQUEST FURTHER DOCUMENTATION IF NECESSARY.
THE DOCUMENTATION SUBMITTED WILL BE KEPT BY THE EMBASSY EVEN IF VISA IS
REFUSED.
ALL DOCUMENTS IN RUSSIAN/UKRAINIAN LANGUAGE MUST BE SUBMITTED ALONG WITH AN
ITALIAN/ENGLISH TRANSLATION OF THE SAME.
