                                                       Documents required for a Tourism Visa

 

An agreement on facilitation of visa requirements between EU and Ukraine has been in force since January 1, 2008. Due to the fact that in accordance with this agreement, the documents, required to confirm the purpose of the trip for certain citizen categories, may differ from the general list indicated   below, we ask you to consult the instructions on the site of the Delegation of the European Commission in Ukraine.

1.       Visa application form

2.       Two recent passport-size photos.

3.

  • Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
  • Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
  • Copy of internal passport.

4.     Reservation of a return-ticket or a document acknowledging the existence of a personal vehicle (Green Card, driving license and car passport)

5.       Proof of accommodation in Italy :

·   or tourist agency voucher or hotel booking reservation

·   or documentation that proves rent of lodging

·   or orinvitation letter along with copy of ID of inviting person (including page with the signature of mentioned person)

6.      Documents confirming personal financial support in accordance with the Directive of the Ministry of Interior Affairs of Italy of 01.03.2000 (click here)

·   or traveler's cheques

·   or a bank guarantee

·   or current bank account slip with a copy of the bank card or a credit card

7.    Close relatives - husband/wife, children (particularly adopted), parents (particularly adoptive parents, custodians), grandparents, grandchildren who are intended to visit citizens of Ukraine which are legally residing in Schengen countries should submit:

·   written invitation letter

·   copy of residence permit (Рermesso di soggiorno)

·   documents that prove relative ties (birth certificate, marriage certificate etc.)

          Documents to prove your socio-professional state

·   Reference from employer and labor book (copy)

·   Reference from university and transcript book (copy)
Copy of student's card

          If employer of applicant is a private entrepreneur:

·   Registration certificate

·   Work agreement assured in tax administration

          Private entrepreneurs are to present:

·   Registration certificate

·   Income certificate      

        Applicants of liberal profession are to present documents that prove activity in stated branch (e.g. license of activity, income           certificate).

        For pensioners - pension card

8.   A medical insurance policy with a minimal coverage of 30.000 euro for urgent hospitalization and repatriation to homeland

Note:

  • The Diplomatic and Consular Representative Offices reserves the right to demand any additional documents
  • All the documents of Ukrainian origin must be translated into Italian or English
