Documents required for SELF Employment Visa
Sports persons and athletes

Visa application
form

Two recent passport-size photos. (3,5 x 4,5 cm).

- Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
- Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
- Copy of internal passport.
4.
Name permission from Italian National Olympic Committee
given to the competent Italian Diplomatic/Consular Representative. Type of sport
activity,, the athlete’s personal data, details and address of the firm of destination
must be indicated in the permission
5.
Nulla Osta of Questura issued no more than 90 days earlier
