Visa application form
- Two recent passport-size photos (3,5 x 4,5 cm).

- Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
- Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
- Copy of internal passport.
- A declaration issued by competent Italian body (professional
associations), that certifies the right of the applicant to carry out the corresponding
professional activity.
- A documentary proof of the available habitation that can
be submitted in the following ways:

A purchase or rental contract
·
A statement in accordance with article 2 and 4 of the Law #15 of 4.1.1968
·
A statement of an Italian citizen or a foreigner residing in Italy of giving an
accommodation that meets the minimal parameters provided by laws and regulations of the public housing construction.
- Documents on the income earned in
the country of residence
for the last accounting period higher than the minimum level provided
by the legislation
for exemption from participation in health services
contributions (€8,400)
- Permission (Nulla Osta) of the competent Questura valid
within 90 days.

Diplomatic and Consular Representative Offices
reserves
the right to demand any additional documents.

All the
documents of Ukrainian origin must be translated into Italian or English.
