                                                           Documents required for Self-employed Visa

1.       Visa application form

2.       Two recent passport-size photos (3,5 x 4,5 cm).

3.      

  • Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
  • Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
  • Copy of internal passport.

4.       Job contract with the original signature of supervisor (the holder of the license to operate, impresario or legal representative) ensuring the self-employed person compensation is higher than the level of salary of national contracts subordinate employers of the same qualification

5.       Appropriate professional documentation, issued by competent authorities in the country of residence and validated in the Italian Consular Authority

6.       Certification of the firm's enrolment in the Business Registry

7.       Documentation which proves that company’s income is sufficient to guarantee compensation of the applicant mentioned in point 4 above: (for limited companies - copy of the latest balance sheet filed at the Business Registr, for companies and free enterprises - the latest income declaration)

8.       Availability of suitable lodgings can be proven by one of the following means:

·         Rental lease or ownership deed

·         Declaration in keeping with articles 2 and 4 of Law no. 15 of 4.1.1968

·       Declaration of Italian or foreign citizen legally residing in Italy of providing visa applicant suitable lodgings that fulfill the minimum requirements foreseen by regional legislation for public housing

9.      Copy of the declaration of responsibility sent by representative of the company to competent Labor Inspection Service of the Italian DPL and to Provincial Labor Office indicating that in virtue of the contract the applicant will not be employed as a subordinate worker

10.   Nulla Osta of Questura issued no more than 90 days earlier

  • The Diplomatic and Consular Representative Offices reserves the right to demand any additional documents
  • All the documents of Ukrainian origin must be translated into Italian or English
