Documents required for Self-Employment
Visa

1.
Visa application form
2.
Two recent passport-size photos (3,5 x 4,5 cm).

- Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
- Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
- Copy of internal passport.
4.
A documentary proof (certification) of the available means
for entrepreneurial, commercial or artisan activity, issued by the Competent Chamber of
Commerce and Industry on the territory where the activity is executed.
These means should not be less than the amount of the annual social payments (4.962.36
euro).
5.
A documentary proof of the available financial means in
Italy
in the amount not less than indicated in paragraph 4.
6.
A documentary proof of the available habitation that can
be submitted in the
following ways:

A purchase or
rental contract
·
A
statement in accordance with article 2 and 4 of the Law #15 of 4.1.1968
·
A
statement of an Italian
citizen or a foreigner residing in Italy of giving an accommodation
that meets the minimal parameters provided by laws and regulations of the public housing construction.
7.
Documents on the income earned in the country of residence
for the last accounting period higher than the minimum level provided
by the
legislation for exemption from
participation in health services contributions (€8,400)
8.
Permission (Nulla Osta) of the
competent Questura.
