Documents Required

Since new Code of Conduct has been implemented, new requirements to be followed by applicants who would like to apply for visa at this Embassy:

А. General requirements for all visa categories:

• Visa Application Form (fully filled and signed)
• 2 coloured photos (recent) of passport size (3.5 * 4.5 sm)
• Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
• Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
• Copy of internal passport.
• Medical Insurance policy (validity should be 15 days more than planned trip), if this requirement has not been cancelled

General additional requirements for minors (below 18):

• Written permission from both parents whether they accompany minor during the trip or not. Written permission should be issued at notary (copy to be submitted).
• Copies of internal passports of parents
• Birth certificate – original and copy
• For schoolchildren: reference from school.

Special cases

• In a case when one of the parents is absent - judge decision of deprivation of parental rights.
• If one of the parents has died - death certificate
• If child lives with single mother, reference according to article 135 of Family Law
• In a case of custody: decision of judge or other competent institution.
• If minor is travelling alone - notarized permission from parents or person who has corresponding rights
