1. Visa application form
2. Two recent passport-style photographs
3. Passport or travel document valid for at least three months after visa expiry date
4. Invitation letter with a copy of the Identity Card from the sponsor in Italy.
5. Visura Camerale (date should be less than 3/6 months regarding to SPA or other organization
6. The contract between the customer & the transport company (that must be registered by the 'Camera di Commercio' in Italy)
7. Letter from employer in Ukraine (petition)
8. Work Agreement (given by driver's company)
9. Copy of the first & last page of theWork-Order book translated into Italian
10. Copy of Driver's License
11. Copy of the Ukrainian transport company's Certificate translated into Italian
12. Copy of certificate of Registration of Ukrainian Company translated into Italian
13. Proof of economic means of support as per Ministry of Interior Directive 1.3.2000 (travelers cheques or credit card)
14. Health insurance with a minimum coverage of ? 30,000 for emergency hospital and repatriation expenses
15. Original & copy Carnet Tir
16. Original custom's invoices in the case of previous unloading in Italy called CNR
17. Trips's permission issued by Ministry of Transport of Ukraine
18. Original & copy of previous passport
