Documents required for a Transport Visa

 

An agreement on facilitation of visa requirements between EU and Ukraine has been in force since January 1, 2008. Due to the fact that in accordance with this agreement, the documents, required to confirm the purpose of the trip for certain citizen categories, may differ from the general list indicated below, we ask you to consult the instructions on the site of the Delegation of the European Commission in Ukraine.

1.     Visa application form

2.        Two recent passport-size photos (3,5 x 4,5 cm).

  • Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
  • Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
  • Copy of internal passport.

4.        Labor book (original and copy of all filled pages) and driving license of international standart

5.    Written request (in original) on behalf of National Transport Ukrainian Associations concerning international  transportations (ASMAP, SE SIRC, URTLU, AAAC) where destination, period of stay and frequency of trips on the  territory of Italy or other EU countries, information of car passport are mentioned

6.   Documents confirming personal financial support in accordance with the Directive of the Ministry of Interior Affairs of Italy       of 01.03.2000 (click here)

·         or traveller’s cheques

·         or bank guarantee

·         or current bank account with a copy of the bank card or a credit card

7.   A medical insurance policy with a minimal coverage of 30.000 euro for urgent hospitalization and repatriation to       homeland

Note:

  • The Diplomatic and Consular Representative Offices reserves the right to demand any additional documents
  • All the documents of Ukrainian origin must be translated into Italian or English
