                                                   Documents required for a Business Visa

                                                    Members of TV, radio or cinema crews

 

  1. Visa application form
  2. Two recent passport-size photos (3,5 x 4,5 cm).
    • Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
    • Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
    • Copy of internal passport.

  3. Reservation of a return-ticket or a document acknowledging the existence of a personal vehicle (Green Card, driving license and car passport).
  4. A letter of inquiry from an authorized local organization with indication of the purpose of the trip
    • Journalists who work as employees have to submit a certificate of professional occupation issued by one of the following associations (National Ukrainian Journalists Union, Independent Media Trade Union of Ukraine, Kyiv Independent Media Trade Union, The Ukrainian Press Academy, The Regional Press Development Institute, The Ukrainian Cultural Workers Trade union) that proves activity of applicant as journalist in Italy.
    • Document issued by the employer that explains the aim of the trip, the program of the visit and the address of residence in Italy.
  5. Documents confirming personal financial means of support in accordance with the Directive of the Ministry of Interior Affairs of Italy of 01.03.2000 (click here)

·         or bank guarantee

·         or current bank account with a copy of the bank card or a credit card

  1. An insurance medical policy with a minimal coverage of 30.000 euro for urgent hospitalization and repatriation to homeland.

 

Note:

  • The Diplomatic and Consular Representative Offices reserves the right to demand any additional documents.
  • All the documents of Ukrainian origin must be translated into Italian or English.
