Documents
required for a Business Visa
An agreement on facilitation
of visa requirements between EU and
Ukraine
has been in force since January 1, 2008. Due to the fact that in accordance with
this agreement, the documents, required to confirm the purpose of the trip for certain
citizen categories, may differ from the general list indicated
below, we ask you to consult the instructions on the site of the
Delegation of the European Commission in Ukraine.

Visa application form

Two recent passport-size photos (3,5 x 4,5 cm).

- Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
- Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
- Copy of internal passport.

Reservation of a return-ticket or a document acknowledging
the existence of a personal vehicle (Green Card, driving license, car passport)

) Documents that confirm the financial and commercial status of the applicant:

Copy of labour-book (attested by original of official seal of the company and signature of responsible person);

A certificate from the place of employment with indication of the position and the
wage amount.
b) Private entrepreneurs
are to submit:

Documents for entrepreneurship (e.g. registration certificate, income certificate).
6. An original of
letter of invitation from Italian organization
along with copy of ID of person who signed the
invitation (copies of 1 and 2 pages of passport with signature of passport
holder or Carta d'Identita) and certificate from Chamber of Commerce
about the registration of inviting company (issued not earlier than 6 months). Use the example at

Documents confirming personal financial support in accordance with the Directive of the Ministry of Interior Affairs of Italy of 01.03.2000 (click
here)

or bank guarantee
·
or current bank account with a copy of the bank card or a credit card
8.
A medical insurance policy with a minimal
coverage of 30.000 euro for urgent hospitalization and
repatriation to homeland.
Note:

The
Diplomatic and Consular Representative Offices
reserves
the right to demand any additional documents.

All the
documents of Ukrainian origin must be translated into Italian or English.
