ACCOMPANYING FAMILY MEMBER - JOINING FAMILY MEMBER
Of a foreign citizen with residence permit or work visa for subordinate
employment, self-employment, study or religious reasons valid for no less than
one year, as well as residence permit for Asylum and Familiy Purposes

Visa application form

One recent passport-style photograph
-
Passport or travel document valid for at least three months after visa expiry
date

"Nulla osta" (entry clearance) issued no more than 6 months earlier by the
competent "Sportello Unico" (Unified Immigration Desk)

Administrative documentation (marriage certificate, birth certificate,etc.)
attesting to the familial relationship, also economic guarantees, medical
certificate. If applicant elder than 18 years old and below 65he/she should
proof a financial dependence.

In the case of minor children, written permission from the child's other parent
(valid for 6 months) for a long stay visa to be issued. If one of the
parents is absent it is necessary to present court's decision about deprivation
of parent's rights. If one of the parents is died it is necessary to present a
death certificate, if a single mother, certificate from the registry office,
that information about father was written according to the mother's words.

"Nulla Osta" is not required. The document which is in force called -
"Dichairazione" - filled by the citizen of EU or EEA

Administrative documentation (marriage certificate, birth certificate, etc.)
attesting to the familial relationship. If applicant elder than 21 years old,
it is necessary to present a document, which confirms financial dependence from
the EU citizen or EEA (example: Western Union).

LEGALIZED BY TRANSLATION IN THE EMBASSY OF ITALY
