Documents required for a FAMILY RE-UNION Visa or Visa FAMILIARE AL SEGUITO with EU citizen

  1. Visa application form

  2. Two recent passport-size photos. (3,5 x 4,5 cm)

    • Signed passport, with expiry date at least three months later than planned visa duration. Passport should have at least 2 blank pages (for visa and stamps of custom authorities) and must be issued not earlier than 10 years ago (p. 12 of Code of Conduct). Extended passports will not be accepted, except for urgent humanitarian cases which can be proved.
    • Originals of all valid international passports and expired passports in original and copies, in case applicant has them.
    • Copy of internal passport.

  3. Declaration filled by citizen of EU or EEA, for family re-union according to the requirements of valid legislative law. Copy of ID document of EU citizen.

  4. Documentation that proves relative ties between the person who makes request for family re-union and applicant (for example marriage certificate, birth certificate, reference of family status). Document provided MUST be issued in the last 6 months with an Apostille stamp (document should be translated in Italian language and legalised)

    For any document issued outside of Ukraine, legalization of such documents MUST be done by the Italian Embassy/Consulate in the country where such document was issued.

    Special Occasions

  5. For applicants elder than 21 and below 65, it is necessary to present corresponding documentation that proves financial   dependency on the person who makes request for family re-union (western union cheques, other receipts or money   transfers). Inquiry which indicates that applicant is unemployed.

  6. For applicants with special needs – medical inquiry given by the doctor accredited by Consular Chancellery.


  7. For minors – notarized permission from parents to go to Italy for long stay (Document must be issued and notarised within the last 6 months)

    If such letter is issued in Italy: Notarisation required in Italy by Italian notary

    If such letter is issued in Ukraine: Notarisation required from Ukrainian Notary; Apostille stamp by Italian Embassy/Consulate; Translation in Italian with legalisation stamp

    If such letter is issued in countries other than Italy and Ukraine: Notarisation and Apostille stamp required from the Italian Embassy / Consulate in the country where such document is issued; Translation in Italian with legalisation stamp by competent authority (i.e. Italian Embassy/Consulate or Ministry of Justice)

  8. In a case when one of the parents is absent - judge decision of deprivation of parental rights or death certificate. (Such document must be issued in the last 6 months with Apostille stamp. Italian translation with legalisation is also needed)

    For any document issued outside of Ukraine: Apostille stamp required from the Italian Embassy / Consulate in the country where such document is issued; Translation in Italian with legalisation stamp by competent authority (i.e. Italian Embassy/Consulate or Ministry of Justice)

  9. If child lives with single mother - reference according to article 135 of Family Lawbook. of Ukraine (Such document must be issued in the last 6 months with Apostille stamp. Italian translation with legalisation is also needed)

Visa type FAMILIARE AL SEGUITO foresees trip of the applicant to the country of destination accompanied by family member – EU citizen who makes request for family re-union

 

Documents required for a FAMILY RE-UNION Visa or Visa FAMILIARE AL SEGUITO with foreign citizen who has residence permit for subordinate employment, self-employment, study, religious activity that is valid not less than one year and permission for asylum and family reasons.

  3. Permission (Nulla Osta) issued no more than 6 months before by competent Unified Immigration Desk ("Sportello Unico") of Prefecture of residence of person who makes request

  4. Documentation that proves relative ties between the person who makes request for family re-union and applicant (marriage certificate, birth certificate, reference of family status(for example marriage certificate, birth certificate, reference of family status). Document provided MUST be issued in the last 6 months with an Apostille stamp (document should be translated in Italian language and legalised)

    For any document issued outside of Ukraine, legalization of such documents MUST be done by the Italian Embassy/Consulate in the country where such document was issued

Family re-union visa category is foreseen for the following relatives :

a)       Full aged husband or wife who is officially married.

b)       Minor children of husband or wife born out of the wedlock that live together in chime of both parents.

c)     Full aged children who are financially dependent on parents and for objective reasons have no opportunity to provide oneself or in case of full unemployment (inquiry of disability is required). Corresponding level of disability can be proved by accredited doctor of the Embassy who is responsible, if needed, for appointing Special Medical Board (all expenses must be covered by applicant).

d)     Parents who are financially dependent and have no other children in homeland or parents who are older than 65, in case if their children for objective reasons cannot provide them (for health reasons) that can be proved documentally.

 

Note:

  • The Diplomatic and Consular Representative Offices reserves the right to demand any additional documents.
  • Documentation that proves relative ties, documents of civil statuses, permission from parents, judges of court must be issued in the last 6 months
  • Documentation that proves relative ties, documents of civil statuses, judges of court, permission from parents (for minors), medical documentation (for applicants with special needs) must have apostille and legalized translation
